Insist on a smoking room to begin with.

If it's a totally non-smoking hotel, find a more smoker-friendly hotel.

And if all of that is not possible, carry a small portable ashtry.

It was some sort of service elevator, the guy was pretty much screwed. The answer is always carry a cell phone I think. It doesn't matter if you're just going out for a smoke.
